What can be discovered in connection to painful bowel obstruction?

A 24-years old female patient from US with status after multiple laparotomies and extreme pain in the middle and right upper abdominal quadrant since 3 months. Permanent pain medication (Nubain injections) and unclear amenorhea since 12 weeks where she was told, it is due to narcotics. Our pre-operative sonography discovered a pregnancy of 12 weeks age. Clinically a big bowel obstruction was evident with massive megacolon transversum and descendens.
